SPRING SOCCER - LATE REGISTRATION
Mail-In Registration

Instructions

1. Click on the link for ONLINE REG FORM on the left side of the page to begin after you have read these instructions.
2. Click on LAUNCH ONLINE FORM at the top of the page.
3. The application will prompt you for email id and password. You must first Register to use the website and to obtain a log on password. The website will send you your initial password in a separate email.
4. Use your email address and sent password to logon to the website and change your password if you desire.
5. The application has menus on the left side of the screen to assist you.
6. Under the FAMILY option, add the Parent contact information and then add your Child(ren)'s information.
7. Under the REGISTRATION option, choose Complete Form to walk-thru the steps to complete and print your registration form.
8. The completed form will display in your browser and will prompt for a printer. For future Fall/Spring registrations, you will only need to go to Complete Form, unless you need to update family information.
9. After printing click on the BACK link on the top right-side of the form to return to the application.
10. Make the check out to "LVSA" for the total amount of all the form(s) you will be mailing.
11. Mail the printed form(s) with full payment to:

I must receive your registration no later than February 28, 2010 because the program closes on 3/1/2010, no exceptions!
